Fleet requires ORIGINAL copies of the applications listed in red.
By submitting one of the forms below, I agree to comply with the guidelines specified in the Fleet Handbook and the Fleet Policies and Procedures. I further understand that the vehicle I am driving will be monitored electronically at any time at Fleet Services’ discretion.
- Authorized Driver Designation Application
- Authorized Driver Designation Application - Group Home Only
- Automobile Accident Form
- Exemption from "State Owned" License Plate Designation
- Contractual Driver Questionnaire – Must be submitted in addition to the Authorized Fleet Driver Application for those individuals who are contractual/interns/volunteers/temps/seasonal.
- Misuse/Abuse of a State Vehicle Form
- PHRST: Vehicle Usage Policy and Procedure – Link is only available from within the State domain.
- Request for a Blocked Vehicle
- Vehicle Housed at Alternate Location Application
- Vehicles Used by Contracted Workers policy
Agency Vehicle Turn In
Surplus Services is now accepting vehicle turn-ins at the Mid-Del lot.

We will be receiving vehicles Monday - Friday from 9am-2pm at the Mid-Del Distribution Center.
You must contact the Surplus office (302-836-7640) to schedule prior to turn-in so we know when to expect you.
Please remember:
- A completed Title must accompany vehicle, or have been received by Surplus Services prior to turn in.
- A completed Vehicle Turn in form, noting actual mileage at time of turn-in and any mechanical defects, should be placed on the dash of vehicle.
- All License Tags, Agency/District decals, Agency/District names, and EZ-Pass transponders should be removed from vehicle prior to turn-in to Surplus Services.
- If reimbursement is required, a fully completed Equipment Reimbursement Form (AM001) must also be included with vehicle.
